Job summary

Sweetwater Care is seeking a licensed LVN/LPN to provide nursing care within scope of practice, ensuring patient needs are met per standards, orders, and policy. You will document changes, participate in rounds with physicians, and follow narcotic and incident-reporting procedures.

The role emphasizes applying state guidelines and center policies while supporting care quality and patient safety in a long-term care setting.

Qualifications

• LPN/LVN licensed in the practicing state.
• BLS/CPR certification or willingness to obtain one.
• Experience in long-term care settings is required or preferred.

Responsibilities

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including nutritional issues, infections, skin conditions, weight loss, or other noticeable changes.
• Uses the Quality Life Manual to identify, place, and monitor patients in appropriate programs.
• Documents and notifies applicable departments of room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
