Job summary

Sweetwater Care is seeking a licensed LVN/LPN to provide nursing care within the scope of practice, ensuring patient needs are met in accordance with standards, physician orders, center policies and applicable guidelines. The role requires long-term care experience, familiarity with PointClickCare charting, and current licensure in the practicing state, plus BLS/CPR certification or willingness to obtain.

Qualifications

• Long-term care experience required.
• PointClickCare charting experience.
• Valid LVN/LPN license in California.
• BLS/CPR certification or willingness to obtain.
• Nursing management experience preferred but not required.

Responsibilities

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents changes in patient conditions including nutrition issues, infections, skin conditions, weight loss, or other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in appropriate programs.
• Documents and notifies applicable departments of patient room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
