Job summary

Diablo Valley Post Acute is seeking a compassionate Licensed Vocational Nurse (LVN) to join our dedicated nursing team. This part-time role provides direct patient care and supports CNAs under the supervision of the Director of Nursing.

The LVN will administer medications, monitor residents, document care plans, and help maintain safety and quality in a fast-paced skilled nursing environment. We offer competitive hourly pay and benefits, plus growth opportunities.

Qualifications

• Current and active LVN license required
• Skilled nursing or post-acute care experience preferred
• Strong communication and organizational skills
• Ability to work effectively in a fast-paced healthcare environment
• Compassionate, resident-focused approach to care
• Team-oriented mindset and strong clinical judgment

Responsibilities

• Provide direct nursing care and treatments per physician orders and care plans
• Administer medications and monitor resident responses
• Observe and report changes in resident condition to nursing leadership and physicians
• Supervise CNAs during assigned shifts and ensure safe care
• Maintain accurate and timely clinical documentation
• Promote a positive, collaborative work environment for the nursing team
• Follow all federal, state, and facility regulations and policies
