Job summary

Tampico Healthcare Center in Walnut Creek, CA is seeking a dedicated LVN to join its 128-bed skilled nursing rehabilitation team. You will work under the supervision of a registered nurse to implement the care plan for residents, with a per diem schedule offering flexibility and autonomy.

Requirements include CA LVN licensure, BCLS certification, and LVN graduation from an accredited school. At least six months of prior LVN experience in a general acute care setting is preferred.

Qualifications

Responsibilities

• Provide total care for chronically ill and medically dependent residents, including medication administration and treatments.
• Assist with planning, coordination, and provision of individualized resident care per established policies.
• Collaborate with nursing staff, physicians, and managers to ensure high-quality care.
• Provide resident assessment education and documentation.
• Maintain accurate and thorough records of resident care.
