Tampico Healthcare Center in Walnut Creek, CA is seeking a dedicated LVN to join our team of skilled nursing professionals. The LVN will work under the supervision of a registered nurse to implement the plan of care for residents in our 128-bed center.

Qualifications

• CA LVN license required.
• BCLS certification required.
• LVN program graduate from an accredited school.
• Minimum six months LVN experience in acute care.
• Strong reading comprehension and English communication.

Responsibilities

• Provide total patient care for chronically ill residents.
• Assist planning, coordination and provision of individualized care.
• Collaborate with nursing staff, physicians and managers.
• Provide resident assessment-related education and documentation.
• Maintain accurate resident care records.
