Job summary

LoveWell Hospice is seeking a compassionate LVN Patient Care Coordinator in Allen, TX to serve as the coordination hub for the clinical team. You will ensure timely visits, accurate documentation, and regulatory compliance while delivering exceptional service to patients and families.

The role requires strong communication, empathy, and reliability in a fast-paced hospice environment, with opportunities to collaborate across nurses, aides, social workers, and leadership.

Qualifications

• Current, active Texas LVN license.
• Strong organizational and time-management skills.
• Excellent verbal and written communication.
• Proficiency with EMR/EHR systems and Microsoft Office.
• Ability to manage multiple priorities with attention to detail.

Responsibilities

• Care Scheduling: coordinate schedules for call-outs and PRN coverage.
• Visit tracking: monitor HOPE visits and timelines.
• IDG Prep: compile and generate IDG reports.
• Chart Accuracy: update patient charts for field staff.
• Visits Auditing: ensure plan-of-care compliance.
• Utilization Management: track level of care changes.
• Intake & Triage: handle urgent calls and route to clinicians.
