Touchstone Communities in Texas is seeking an LVN or RN with MDS experience to join our skilled nursing team as an MDS Coordinator. You will oversee the MDS assessment and reimbursement process, ensuring accuracy, regulatory compliance, and high-quality resident care.

Qualifications

• Strong understanding of Medicare, Medicaid, and case management reimbursement.
• Experience in Skilled Nursing Facilities (SNF) as an MDS Nurse preferred.
• Knowledge of state and federal regulations related to RAI and reimbursement.
• Detail-oriented with excellent organizational and communication skills.
