Job summary

Medical Hill Healthcare in Oakland, CA is seeking a licensed vocational nurse (LVN) to join our nursing team on an on-call/per diem basis. You will provide direct nursing care, administer medications, monitor residents, and collaborate with CNAs, RNs, physicians, and the interdisciplinary team.

The ideal candidate holds a California LVN license and CPR certification, with prior skilled nursing or long-term care experience.

Responsibilities

• Assist with admissions, transfers, and discharges as needed.
• Communicate effectively with residents, families, and the interdisciplinary team.
• Provide wound care and other LVN scope treatments.
• Monitor vital signs and other clinical indicators.
• Maintain accurate medical records.
• Follow infection prevention and control procedures.
• Respond promptly to needs, emergencies, and call lights.
• Ensure compliance with policies and regulations.
