Job summary

Medical Hill Healthcare in Oakland, CA, is seeking a compassionate LVN for an on-call/per diem role. The LVN will provide direct nursing care, administer medications, and work with CNAs, RNs, physicians, and the interdisciplinary team to support residents’ safety and comfort.

Qualifications

• Active California LVN license required.

Responsibilities

• Provide direct nursing care to residents per care plans.
• Administer medications and treatments as ordered.
• Monitor residents and report changes to RN/physician promptly.
• Perform nursing assessments and document care accurately.
• Assist with admissions, transfers, and discharges.
• Communicate with residents, families, and interdisciplinary team.
• Provide wound care and other LVN scope tasks.
• Monitor vital signs and clinical indicators.
• Maintain medical records and follow infection control procedures.

Position Responsibilities

• Provide direct nursing care to residents according to individualized care plans
• Administer medications and treatments as ordered
• Monitor residents' conditions and promptly report changes to the RN or physician
• Perform nursing assessments and accurately document resident care
• Assist with admissions, transfers, and discharges as needed
• Communicate effectively with residents, families, and members of the interdisciplinary team
• Provide wound care and other treatments within the LVN scope of practice
• Monitor vital signs and other clinical indicators
• Maintain accurate and timely medical records
• Follow infection prevention and control procedures
• Respond promptly to resident needs, emergencies, and call lights
• Ensure compliance with facility policies, state and federal regulations, and standards of nursing practice
• Promote resident dignity, safety, comfort, and quality of life

Qualifications

• Current and active California LVN license required
• Current CPR certification required or preferred
• Previous skilled nursing, post-acute, or long-term care experience preferred
• Strong clinical assessment and communication skills
• Ability to work independently and effectively as part of a team
• Dependable, compassionate, and resident-focused
• Strong organizational and time-management skills
• Ability to work flexible on-call shifts as needed
