Job summary

PACS Group is seeking an LPN/LVN supervisor to provide direct nursing care and oversee day-to-day nursing assistant activities in a long-term care facility. You will ensure compliance with federal, state, and local regulations and collaborate with the Director of Nursing to maintain high-quality care.

You will lead the nursing team, participate in QA activities, manage discharge planning, and ensure accurate charting and reporting. A valid LPN/LVN license and CPR certification are required.

Qualifications

• Must possess a Nursing Degree from an accredited college or university, or be a graduate of an approved LPN/LVN program.
• Must have a current, active license to practice as an LPN/LVN in this state.
• Knowledge of PCC preferred and CPR certification.
• Ability to read, write, and understand English; strong communication with staff and families.

Responsibilities

• Direct the daily nursing Assistant functions in accordance with current regulations and facility policies.
• Supervise nursing staff, participate in shift planning, and maintain quality assurance procedures.
• Review and update care plans, discharge plans, and physician orders as required.
• Ensure compliance with documentation standards and reporting requirements.
• Coordinate with other departments to ensure residents receive comprehensive care.
