Job summary

Oakwood Gardens Care Center in California is seeking a Licensed Vocational Nurse (LVN) for the Full-Time NOC Shift. The LVN provides direct nursing care to residents and supervises CNAs under the direction of the DON to ensure high-quality resident care.

This role administers medications and treatments, documents care, monitors resident status, and participates in admissions, transfers, and discharges. It requires strong clinical, leadership, and communication skills, plus compliance with HIPAA

Qualifications

• Graduate of an approved LVN/LPN program.
• Long-term care or skilled nursing experience preferred.
• Current, active, and unencumbered LVN license in the State of California.
• Current CPR certification.
• Knowledge of PCC preferred.

Responsibilities

• Provide direct nursing care in accordance with resident assessments and care plans.
• Supervise and support CNAs to ensure quality care delivery.
• Administer medications and treatments as ordered by physicians.
• Accurately document nursing care, medication administration, and resident status.
• Monitor residents for changes in condition and report findings promptly.
• Participate in admissions, transfers, and discharges as assigned.
• Ensure compliance with infection control, safety, and sanitation standards.
• Communicate effectively with residents, families, physicians, and interdisciplinary team members.
• Assist with restorative and rehabilitative nursing programs.
• Respond appropriately to emergencies and resident needs.
• Maintain resident confidentiality and comply with HIPAA.

Job Title

Licensed Vocational Nurse (LVN) - Full-Time NOC Shift

Facility: Oakwood Gardens Care Center

Shift: NOC Shift

Salary Range: $28.00 - $30.00 per hour (BOE)

General Purpose

The Licensed Vocational Nurse (LVN) provides direct nursing care to residents and supervises the day-to-day nursing activities performed by Certified Nursing Assistants (CNAs). Care is delivered in accordance with current federal, state, and local regulations, facility policies, and under the direction of the Director of Nursing (DON) or Nurse Supervisor to ensure the highest quality of resident care.

Qualifications

Education & Experience

• Graduate of an approved LVN/LPN program


• Long-term care or skilled nursing experience preferred

Licenses & Certifications

• Current, active, and unencumbered LVN license in the State of California


• Current CPR certification


• Knowledge of PCC preferred

Physical Demands
