Description
Primary Responsibilities

Responsible for coordinating the Resident Assessment Instrument (RAI) process to ensure accurate and timely completion of resident assessments in compliance with Medicare, Medicaid, OBRA, and other payer requirements. Ensures assessments reflect the residents' physical, mental, and psychosocial status and are properly documented. Communicates effectively with the interdisciplinary team and adheres to RIHS policies and procedures.

Essential Functions
  • Ensure timely, accurate, and complete resident health and functional assessments.
  • Participate in pre-admission processes to gather essential information for MDS/Case Mix optimization.
  • Complete Medicare/Medicaid case-mix documents accurately and timely.
  • Collaborate with the Director of Rehab to determine appropriate assessment reference dates.
  • Track Medicare/Managed Care eligibility and benefits, and communicate with relevant teams.
  • Gather information for Managed Care Utilization Reviews and ensure compliance with additional Medicare requirements.
  • Perform concurrent MDS reviews to ensure correct RUGs categories.
  • Participate in interdisciplinary team meetings and facilitate care plan development.
  • Complete all MDS assessments timely, including PPS, Admission, Quarterly, and Significant Change assessments.
  • Identify significant status changes and coordinate the Significant Change in Status MDS.
  • Maintain assessment schedules and rectify default days.
  • Audit MDS processes and ensure proper documentation and coding.
  • Ensure timely electronic submission of all MDS data.
  • Review validation, late/missed assessments, and QM/QRP reports, taking appropriate actions.
  • Communicate with the Business Office regarding case mix and reimbursement impacts.
  • Participate in daily and weekly meetings and assist with additional requests.
  • Serve as a resource for RAI and Care Management processes and train new staff.
Other Duties
  • Manage department operations and maintain knowledge of reimbursement regulations.
  • Organize data and communicate regularly with the Care Management team.
  • Support flexible work hours and utilize resources effectively.
  • Demonstrate clinical assessment and documentation skills and work well in a team.

Qualifications
Educational/Training
  • Graduate of an accredited nursing program with current R.N. or L.V.N. licensure.
  • Completion of RIHS-specific MDS/RUGs training within 90 days.
  • Proficiency with office and MDS/RAI software.
  • Strong leadership and coordination abilities.
Licensing
  • Valid licensure in the state of employment.
Experience
  • At least two years of healthcare experience, with MDS, reimbursement, or case management experience preferred.
