Job summary

Bristol Hospice in East Bay, CA – San Jose, CA is seeking a licensed LVN/LPN for PRN day shifts. Provide direct patient care per the State Nurse Practice Act, support end-of-life care and pain management.

The successful candidate will have at least three years of nursing experience, hospice or medical/surgical preferred, and be flexible with hours. Bristol Hospice values compassionate care and a team-based approach to improve patient and family wellbeing.

Qualifications

• Must be licensed as LPN/LVN in CA.
• Minimum of 3 years nursing experience, hospice or medical/surgical preferred.
• Ability to provide direct patient care and end-of-life support.
• Willingness to maintain knowledge of information systems and hospice protocols.

Responsibilities

• Provide direct patient care under Nurse Practice Act.
• Implement current nursing practice based on plan of care.
• Document services accurately and timely to reflect the plan of care.
• Educate patients and families about diagnosis and self-care plan.
• Coordinate hospice services with interdisciplinary team.
• Maintain aseptic technique with equipment and supplies.
• Provide appropriate pain/symptom management.
• Evaluate patients' responses to treatments/medications.
• Participate in quality assessment and performance improvement activities.

Qualifications: An Average Day:

(Includes, but not limited to)

• Provide direct patient care as defined in State Nurse Practice Act, including specialized duties related to end of life care
• Implement current nursing practice following a comprehensive assessment and the plan of care
• Provide accurate and timely documentation of patient services to reflect the plan of care
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care
• Participate in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group; examples include hospice aide, OT, PT, SW, ST, Dietician or Clinical Supervisor
• Use and prepare equipment and supplies effectively, adhering to aseptic technique
• Provide appropriate pain/symptom management
• Evaluate patients response to treatments/medications
• Participate in personal, professional growth and development
• Participate in organization's quality assessment performance improvement teams and activities
• Perform other duties as assigned by the registered nurse
• All other duties as assigned

Requirements:

• Must currently be licensed as an LPN/LVN in the State
• Must have three (3) years nursing experience, community health/hospice or medical/surgical experience is preferred
• Must understand the philosophy of hospice concept and understand needs of the terminally ill
• Must be flexible in work hours
• Must demonstrate a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
