Key Responsibilities

• Provide direct patient care in a long-term care/skilled nursing facility setting
• Administer medications and treatments as directed by the care plan and supervising clinicians
• Monitor and document patient health status, reporting changes to the appropriate clinical staff
• Collaborate with interdisciplinary care teams to support resident well-being and safety
• Maintain valid state-issued LPN licensure and comply with all facility credentialing requirements
• Wear a photo badge and present a valid state or federal issued photo identification at the facility at all times

Required Qualifications

• Active LPN license in the state of Maine
• Current and valid state or federal issued photo identification required for facility access
• Must have completed the original or updated COVID-19 vaccine series, or be able to present a documented religious and/or medical exemption from your employer
• Additional vaccine doses may be required where mandated by law
• Local candidates only

Preferred Experience / Skills

• Prior experience in a long-term care or skilled nursing facility environment
• Familiarity with electronic health records and clinical documentation systems
