Job summary

Bristol Hospice LLC is seeking a compassionate Licensed Vocational Nurse (LVN/LPN) in California to provide direct patient care in end-of-life settings. You will perform assessments, implement plans of care, and educate patients and families on diagnoses and self-care under the guidance of the interdisciplinary team.

The role requires an active LPN/LVN license in the state and at least three years of nursing experience, with a flexible schedule to meet patient needs.

Qualifications

• Must be licensed as LPN/LVN in the state.
• Three (3) years nursing experience preferred.
• Understand hospice philosophy and terminally ill needs.

Responsibilities

• Provide direct patient care as defined in State Nurse Practice Act.
• Implement nursing practice per assessment and plan of care.
• Document patient services accurately and timely to reflect the plan of care.
