Job summary

Sweetwater Care in California seeks an LVN for a 13-week travel contract to provide compassionate nursing care within the scope of practice and in line with physician orders and facility policy.

The role requires current CA LVN licensure, with preferred experience in nursing management and long-term care. Familiarity with PointClickCare charting is a plus, and the nurse will participate in education for patients and families while coordinating with physicians.

Qualifications

• Currently licensed LVN in CA.
• One-year nursing management experience preferred, not required.
• Long-term care experience.
• PointClickCare charting experience.

Responsibilities

• Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.
• Demonstrates knowledge and appropriate use of the Infection Control Manual.
• Participates in the Quality Improvement Program.
• Uses the Quality Life Manual for patient identification, placement, and monitoring in programs.
• Notifies other departments of patient changes (room changes, LOA, new admits, discharge).
• Conducts family and patient education.
• Administers medications timely and according to facility policy.
• Administers treatments timely and according to facility policy.
• Receives, transcribes, and carries out physician orders when allowed by Nurse Practice Act.
• Makes rounds with physicians when necessary.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.
