Job summary

Eastland Sub Acute & Rehabilitation in Baldwin Park, CA is seeking Licensed Vocational Nurses (LVNs) to join our clinical team. This full-time PM shift (3-11pm) offers the opportunity to lead and support CNAs while delivering compassionate care to residents in a skilled nursing setting.

You will administer medications as ordered, participate in care planning, complete charting, and collaborate with physicians.

Qualifications

• Active California LVN license.
• COVID-19 vaccination including booster.
• Knowledge of long-term care resident needs.
• Ability to work in a fast-paced environment.

Responsibilities

• Provide leadership to certified nursing assistants.
• Oversee the care and wellbeing of residents.
• Prepare and administer medications as ordered by the physician.
• Complete charting and treatment plans/physician orders.
• Admit, transfer, and discharge residents as required.
• Make rounds with physicians as requested/necessary.
• Orient and welcome new residents and families.
• Knowledge of Point Click Care (PCC).
• Other duties as assigned.
