Job summary

Hanford Post Acute in Hanford, CA is seeking a Licensed Vocational Nurse (LVN) to provide direct patient care and lead the nursing team. You will supervise CNAs, administer medications, document care, and participate in admissions and care planning to ensure high-quality resident outcomes.

Qualifications

• Valid LVN license in California (unencumbered).
• CPR certification required.
• Minimum 1 year of experience in skilled nursing or long‑term care preferred.
• Good standing with the State Board of Nursing.

Responsibilities

• Provide direct patient care in accordance with clinical guidelines.
• Supervise and support CNAs and nurse trainees on your shift.
• Administer medications and treatments as prescribed.
• Participate in admissions, discharges, and care planning.
• Ensure compliance with care plans and infection control procedures.
• Maintain accurate charting and documentation.
• Communicate with residents, families, and medical professionals.
• Respond to emergencies promptly and assist in QA and staff development.
