Job summary

Angels Care Home Health seeks an LVN to administer skilled nursing care in clients’ homes under RN supervision. You will coordinate with the interdisciplinary team and families to manage ongoing care and referrals, ensuring high-quality home health services.

The ideal candidate is a graduate of an accredited VN program with a current LVN license, CPR and hepatitis profile on file, and at least two years of LVN experience, preferably in home health.

Qualifications

• Graduate of an accredited school of vocational nursing.
• Current state license as an LVN.
• Two years’ LVN experience; home health preferred.
• Proof of current CPR and Hepatitis profile.

Responsibilities

• administer skilled nursing care in clients' homes under supervision of a registered nurse.
• Coordinate care with interdisciplinary team, patient/family and referring agency.
• Document, monitor and report patient status to ensure quality home health services.
