Qualifications

• Valid California nursing license (LVN).
• Minimum 1 year LVN Home Health experience.
• Proficient in taking and interpreting vital signs.
• Full time / Part time positions available.

Responsibilities

• Provide direct patient care in a home setting, ensuring the comfort and well-being of patients.
• Perform evaluations of patients’ functional status and ADLs/IADL; assess muscle function, endurance, coordination, and communication.
• Assess cognitive, psychological and motor aspects; develop individualized intervention plans.
• Evaluate home environment for hazards or barriers to independence.
• Identify equipment needs and environmental adaptations to improve outcomes.
• Collaborate with nursing and multidisciplinary care teams to revise care plans as needed.
• Educate patients and families in rehabilitative care to promote safety and independence.
• Accurately document observations, interventions, and evaluations.
