ComForCare Home Health Care (North San Diego) is seeking a compassionate Licensed Vocational Nurse (LVN) to provide in-home patient care in San Diego, CA. You will work with a supervising RN to implement care plans and ensure quality outcomes.

You’ll administer medications, monitor vitals, educate families, and document care in accordance with state standards. This role offers flexibility and strong team support in a home-based setting.

Qualifications

• Active LVN license in California and current CPR/BLS certification.
• Experience in home health, skilled nursing, or clinical care preferred.
• Strong wound care, medication management, and patient assessment skills.
• Excellent communication, time management, and organizational abilities.
• Reliable transportation and a valid California driver’s license with proof of insurance.
• Compassionate, self-motivated and able to work independently in home care.
• Ability to develop and follow individualized care plans with RN supervision.

Responsibilities

• Provide direct patient care in home settings including wound care and vital signs.
• Administer medications, injections, and treatments per physician orders.
• Develop and follow patient care plans with supervising RN and care team.
• Observe and report changes in patient condition to clinical staff.
• Educate patients and families on care procedures, medications, and health management.
• Maintain accurate clinical documentation per state and agency standards.
• Coordinate with multidisciplinary teams to ensure continuity and quality of care.
