Job summary

Compassionate Home Health LTD in California is seeking a motivated Per Diem Licensed Vocational Nurse (LVN) to provide skilled nursing care to patients in their homes throughout the Greater Sacramento Area.

Under the supervision of the RN Case Manager, the LVN delivers wound care, medication management, patient education, and routine assessments, collaborates with the interdisciplinary team, and adheres to physician orders, plans of care, and agency policies.

Qualifications

• Current LVN license in California required.
• Current BLS/CPR certification required.
• New graduates welcome; training and mentorship provided.
• Minimum one year of clinical nursing experience preferred; new grads considered.
• Wound care experience preferred; training available.
• Experience with skilled nursing visits, medications, patient education, catheter care, and chronic disease management preferred.
• Valid California Driver’s License, current auto insurance, and reliable transportation.

Responsibilities

• Providing skilled nursing visits to patients in their homes according to the established plan of care.
• Performing patient assessments and reporting changes to the RN Case Manager and physician as appropriate.
• Providing wound assessments, wound care, and monitoring patient progress per orders and protocols.
• Assisting with management of patients with complex medical conditions and chronic illnesses.
• Monitoring vital signs, medications, symptoms, and overall patient progress.
• Performing treatments and procedures within LVN scope of practice.
• Reinforcing patient and caregiver education about medications and disease management.
• Collecting and documenting clinical information in the EMR.
• Completing timely visit documentation and clinical updates.
• Communicating with patients, families, physicians, RN Case Managers, and the team.
• Collaborating with the RN Case Manager on changes in patient status and care needs.
• Following infection prevention, safety, and quality standards.
• Participating in agency training and QAPI activities.
