Hiring for a Home Health LVN (Licensed Vocational Nurse) in Calabasas, CA

Join a reputable home health company with 13 years of experience and growing. Currently, we serve a census of 200 patients. As an LVN, you will provide skilled nursing care under the supervision of a registered nurse, including tasks such as medication administration, wound care, patient education, documentation, and team communication.

Responsibilities

1. Direct Patient Care:
   • Implement the Plan of Care initiated by the RN, providing direct care as per the Nurse Practice Act.
   • Provide and document skilled nursing care, including observations, procedures, and patient/family education.
   • Administer medications and assist with pain management per physician orders.
   • Perform wound care, including cleaning, dressing, and monitoring healing progress.
   • Assist patients with activities of daily living (ADLs) such as bathing, dressing, and eating.
   • Monitor vital signs like blood pressure and temperature.
   • Assess patient needs, create care plans, and perform various procedures.
2. Education:
   • High School Graduate
   • LVN Certification/License
