Qualifications

• Completion of a vocational nursing program or equivalent.
• Active, unrestricted LVN/LPN license in the practicing state.
• Minimum 1 year of nursing experience, preferably with older adults.
• Ability to follow clinical protocols and escalate concerns.
• Strong communication and teamwork skills.
• Proficiency with EMRs and adaptability to systems.

Responsibilities

• Provide direct nursing care within scope and directions.
• Triage participants and monitor health status.
• Support care plan interventions and chronic disease management.
• Coordinate care across clinic, home, and external providers.
• Educate participants and caregivers on medications, nutrition, and health needs.
• Maintain accurate EMR documentation and participate in quality initiatives.
