Qualifications

• Experience with EHR and data integrity best practices.

Responsibilities

• Coordinate intake of home health referrals and ensure timely processing.
• Prioritize referrals, manage pipelines, and forecast admissions.
• Verify patient eligibility and benefits for admissions.
• Maintain accurate patient records in the EHR and ensure data integrity.
• Communicate status with families, clinicians, and payers.
