Job summary

Talented Medical Solutions is seeking an LVN Clinical Intake Coordinator for a 13-week home health assignment in Clearlake, CA. The role serves as a critical connection point among patients, families, physicians and care teams to ensure timely coordination of care and smooth transitions into home health services.

The position runs Monday–Friday, 8:00 AM – 5:00 PM with no on-call duties. LVN certification and at least 1 year of healthcare experience are required; additional home health or intake

Qualifications

• Active LVN certification
• Minimum 1 year of healthcare experience
• High School Diploma or GED
• Associate’s or Technical Degree (or equivalent experience)
• 3+ years of healthcare experience, ideally in home health, intake, or care coordination

Responsibilities

• Manage and coordinate incoming referrals, calls, and inquiries from patients, families, physicians, and healthcare partners
• Verify insurance coverage and ensure accurate intake processing
• Coordinate home health referrals and ensure timely patient onboarding
• Track hospitalized patients and facilitate smooth transitions into agency care
• Communicate effectively with nursing, therapy, social work, and physician teams
• Assist with scheduling and staffing coordination based on program needs
• Support operational workflows and ensure continuity of care across disciplines
• Serve as a working coordinator for intake processes and workflow efficiency
