Cibolo Creek Health in Boerne, TX is seeking a Licensed Vocational Nurse (LVN) to join our devoted care team. You will administer resident care, medications, and treatments, documenting accurately and reporting concerns to the supervisor.

Requirements include Texas LVN licensure, 1 year clinical LVN experience preferred, and proficiency with EMR systems (PCC preferred). Commuting to San Antonio area is expected; relocation planned if needed. We offer paid time off, benefits and a retention bonus.

Qualifications

• Licensed Vocational Nurse (LVN) in good standing and currently licensed by the State of Texas.
• Minimum 1 year experience as an LVN in a clinical setting preferred.
• Electronic Medical Record Experience required, PCC preferred.
• Excellent interpersonal skills.
• Reliable transportation.

Responsibilities

• Follow established standards of nursing practices and implement community programs, policies and procedures.
• Conduct resident rounds daily, reporting problems to supervisor and initiating corrective action.
• Administer and document direct resident care, medications and treatments per physicians’ orders and accurately record all care provided.
• Write detailed reports, maintaining precise charting, recording medications, observing symptoms, and taking vital signs.
