Qualifications

• Graduate from an accredited school of Vocational nursing.
• Licensed Vocational Nurse with the State of Texas is required.
• Current license/valid permit to practice in the State of Texas will be required of all certified individuals.

Responsibilities

• Accurate medical necessity screening and submission for Physician Advisor review.
• Care coordination.
• Transition planning assessment and reassessment.
• Implementation or oversight of the transition plan.
• Leading and facilitating multi-disciplinary patient care conferences.
• Making appropriate referrals to other departments.
• Communicating with patients and families about the plan of care.
• Collaborating with physicians, office staff and ancillary departments.
• Identifying and reporting potentially avoidable days and utilization.
• Ensuring timely, concise documentation in the EMR.
