Molina Healthcare seeks a Texas-licensed LVN to join as a Care Manager, serving Medicaid members in Jasper and Livingston, TX. You will conduct home visits, assess eligibility for services, and develop care plans with the member and care network. Travel may be required, and MS Office proficiency is essential.

Qualifications

• Two years health care experience including LTSS and care management.
• Proficiency with Microsoft Office and ability to navigate online portals.

Responsibilities

• Complete comprehensive member assessments within timelines, including in-person home visits.
• Coordinate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, caregivers, physicians and health professionals.
• Monitor care plans to evaluate effectiveness and progress toward goals.
• Promote integration of behavioral health with LTSS and home/community resources.
• Assess medical necessity and authorize waiver services.
• Facilitate ICT meetings for approval/denial of services.
