Job summary

Molina Healthcare is seeking a Texas-licensed RN Care Manager to conduct in-home assessments for Medicaid members in the Jasper area. You will determine waiver eligibility, develop care plans, and coordinate with the care team to ensure optimal outcomes.

Role emphasizes collaboration across disciplines, with travel to member homes and documentation in typical clinical software. Monday–Friday, standard hours, mileage benefits, and a comprehensive benefits package are provided.

Qualifications

• 2+ years health care experience, LTSS preferred.
• Clinical licensure/certification required only if state contract requires it.
• Bachelor's in a health care field may be required in some states.
• Valid driver's license and reliable auto for travel.

Responsibilities

• Perform in-home member assessments within required timelines.
• Facilitate waiver enrollment/disenrollment processes.
• Develop and monitor care plans with members and care teams.
• Coordinate with physicians and caregivers for service implementation.
• Promote integration of behavioral health and LTSS services.
• Assess medical necessity and authorize waiver services.
• Coordinate ICT meetings for service decisions.
• Travel 25–40% locally as required.
