Job summary

Inland Empire Health Plan (IEHP) seeks a qualified care coordinator to screen members for care management needs, document findings, and communicate with the care team using approved screening tools.

The role involves developing individualized care plans with RN care managers and interdisciplinary team members, educating members, advocating for access, and coordinating referrals and authorizations, while contributing to quality improvement efforts.

Qualifications

• Three (3) or more years of care management experience in a health care delivery setting required
• Active, unrestricted LVN license issued by the California Board required
• Experience in an HMO or Managed Care setting preferred

Responsibilities

• Screen members for care management needs and document findings
• Contribute to Individualized Care Plans with RN Care Managers and Interdisciplinary Care Team
• Educate members according to approved plans and materials
• Coordinate access, referrals, and authorizations
• Advocate for members and coordinate health care access with PCPs, specialists, and ancillary services
• Build relationships with providers and community agencies
• Engage members using Motivational Interviewing
• Support quality improvement initiatives and address HEDIS gaps
