Sequoia Transitional Care, a beautiful 99-bed skilled nursing facility in Porterville, CA, is seeking a Licensed Vocational Nurse (LVN) Charge Nurse. New graduates welcome and training provided. Competitive pay and a collaborative team culture support high‑quality resident care.

The LVN Charge Nurse will lead nursing care, supervise CNAs, and coordinate care with physicians and families while ensuring compliance with regulations and clinical excellence.

Formación

• Current California LVN license in good standing.
• CPR certification required.

Responsabilidades

• Provide direct nursing care to residents in accordance with care plans and physician orders.
• Administer medications and treatments safely and accurately.

• Maintain accurate documentation and nursing notes.
• Assist with admissions, transfers, and discharges.
• Communicate with physicians, residents, and family members regarding care.
• Ensure compliance with infection control and safety protocols.

Position Summary

The LVN Charge Nurse provides direct nursing care to residents and supervises the daily activities of nursing assistants. This role ensures quality care is delivered in accordance with federal, state, and facility regulations while maintaining the highest standards of clinical excellence.

Key Responsibilities

• Provide direct nursing care to residents in accordance with care plans and physician orders

Physical Requirements

This position requires standing, walking, lifting, and assisting residents throughout the shift. The ability to lift or move at least 25 pounds may be required.
