Job summary

McClure Post Acute in California is seeking a Licensed Vocational Nurse (LVN) to join our team on a full-time PM shift. You will provide direct nursing care to residents, administer meds, monitor conditions, and collaborate with RNs and other clinicians to uphold quality of care.

Responsibilities

• Provide direct nursing care to residents per physician orders and care plans
• Administer medications, treatments, and therapies safely and accurately
• Monitor resident conditions and report changes promptly to RNs/physicians
• Maintain accurate and timely clinical documentation
• Assist with admissions, transfers, and discharges
• Collaborate with RNs, CNAs, physicians, therapists, and other team members
• Support infection prevention, safety, and regulatory compliance
• Deliver resident-centered care with dignity, comfort, and independence

Key Responsibilities

• Provide direct nursing care to residents in accordance with physician orders and individualized care plans
• Administer medications, treatments, and therapies safely and accurately
• Monitor resident conditions and promptly report changes to the supervising Registered Nurse or physician
• Maintain accurate and timely clinical documentation
• Assist with admissions, transfers, and discharges
• Collaborate with RNs, CNAs, physicians, therapists, and other interdisciplinary team members
• Support infection prevention, safety, and regulatory compliance initiatives
• Deliver compassionate, resident-centered care while promoting dignity, comfort, and independence

Qualifications

• Current and active California Licensed Vocational Nurse (LVN) license
• Current CPR/BLS certification
• Previous experience in skilled nursing, post-acute care, rehabilitation, or long-term care preferred
• Strong clinical assessment, organizational, and communication skills
• Ability to work effectively in a fast-paced healthcare environment
