Lone Tree Post Acute is hiring a Licensed Vocational Nurse (LVN) to join our clinical team in a fast-paced post-acute setting. The role focuses on direct nursing care, medication administration, and collaborating with the interdisciplinary team to ensure high-quality resident outcomes.

Qualifications

• Current and valid California LVN license.
• Current CPR/BLS certification preferred.
• Previous skilled nursing or post-acute experience preferred.

• Ability to work effectively in a fast-paced healthcare environment.

Responsibilities

• Provide direct nursing care to residents in accordance with established care plans and facility policies.
• Administer medications and treatments as ordered.
• Monitor residents’ conditions and report changes to the appropriate healthcare professionals.
• Accurately document nursing care, medications, treatments, and resident progress.
• Assist with developing and implementing individualized care plans.
• Communicate effectively with residents, families, physicians, and members of the healthcare team.
• Maintain compliance with state and federal regulations and facility standards.
• Promote resident safety, dignity, comfort, and overall well-being.
