Lone Tree Post Acute is hiring a Licensed Vocational Nurse (LVN) in California to join our clinical team. The LVN will provide direct nursing care, administer medications, monitor resident conditions, and document care plans.

You will collaborate with residents, families, and an interdisciplinary team to promote safety and wellbeing.

Qualifications

• California LVN license required.
• CPR/BLS certification preferred.
• Skilled in coordinating with interdisciplinary care teams.

Responsabilités

• Provide direct nursing care to residents according to care plans and facility policies.
• Administer medications and treatments as ordered.
• Monitor residents’ conditions and report changes to the care team.
• Document nursing care, medications, treatments, and progress accurately.
• Assist in developing and implementing individualized care plans.
• Communicate effectively with residents, families, and the healthcare team.
• Maintain compliance with state and federal regulations and facility standards.
