Job summary

Caregiver is seeking a compassionate LPN/LVN to join our team and provide hands‑on care to individuals with IDD. You will work under an RN to support daily health needs, administer medications, and ensure a safe, supportive environment.

You will monitor health, document care, and collaborate with RNs and other providers. A valid LPN/LVN license (multi‑state preferred) and prior IDD/long‑term care experience are valued for success in this role.

Qualifications

• Active LPN/LVN license (multi-state preferred)
• Graduate of accredited LPN/LVN program
• 1+ year nursing experience preferred
• Experience with IDD or long‑term care preferred
• Strong attention to detail and observation skills
• Familiarity with EHR systems and basic computer tools

Responsibilities

• Provide routine nursing care and health monitoring
• Administer medications and treatments as directed
• Observe and report changes in health or behavior
• Assist with daily living activities (ADLs) like hygiene, mobility, and nutrition
• Support implementation of individualized care and behavior plans
• Maintain accurate documentation and medication records
• Ensure proper medication storage, handling, and inventory
• Respond to emergencies and provide immediate care when needed
• Collaborate with RNs, providers, and interdisciplinary teams
