Job summary

UNC Health in Chapel Hill/Durham area seeks a Clinical Pharmacist Practitioner for our LVAD program. The CPP will support advanced heart failure care, manage medications, and provide clinical guidance to physicians and staff within a multidisciplinary team.

The role includes teaching pharmacy students and residents, plus collaborating on quality improvement and education initiatives. Academic appointment at UNC Eshelman School of Pharmacy may be available commensurate with experience.

Qualifications

• Completion of PGY2 residency in Cardiology, Transplant, Ambulatory Care, Pharmacotherapy or equivalent experience.
• Three to five years of LVAD or advanced heart failure experience preferred.
• Board certification in Cardiology, Transplant, Ambulatory Care or Pharmacotherapy preferred.

Responsibilities

• Builds a clinical practice with physicians and providers for LVAD patients.
• Titrates guideline-directed therapy and manages anticoagulation and related issues.
• Educates patients, families, and staff on treatment plans and updates.
• Mentors pharmacy students and residents; participates in quality improvement.
