Job summary

UNC Health Care is seeking a highly motivated Clinical Pharmacist Practitioner to join the LVAD program at UNC Medical Center in Chapel Hill. The CPP will act as an Advanced Practice Provider, managing LVAD therapies and coordinating care with cardiology and transplant teams.

The role includes education and mentorship of pharmacy students and residents, contribution to quality improvement initiatives, and collaboration with UNC Pharmacy divisions to optimize patient outcomes.

Qualifications

• Bachelor's degree in Pharmacy required.
• North Carolina Pharmacist licensure required (CPP) and credentialing/privileging through UNC accrediting body.

Responsibilities

• Clinical practice: partner with physicians to manage LVAD patients and monitor therapy.
• Distributive coordination with SHD Pharmacy and UNC Retail Pharmacies for medication access.
• Precepting/teaching UNC Eshelman School of Pharmacy students and residents.
• Leadership: lead quality improvement initiatives and provider education.
