Job summary

Cleveland Clinic is seeking an experienced Advanced Practice Provider to support care for LVAD patients within the Transplant Center. The role includes inpatient rounds, outpatient visits, and occasional travel to local facilities.

Expect a fast-paced environment with a mix of inpatient and outpatient responsibilities. Responsibilities include history taking, exams, diagnostics, prescribing, patient education, and collaboration with a multi-disciplinary team.

Qualifications

• MSN required upon hire or within 3 years for APRN role.
• Graduate of an accredited APRN program.
• National certification as a Nurse Practitioner.

Responsibilities

• Conducts thorough medical histories and physical examinations to evaluate illness and develop a treatment plan, collaborating with the health care team.
• Orders and interprets labs, radiology and other diagnostic procedures as credentialed and privileged by the Medical Executive Committee.
• Prescribes and monitors medications as allowed by state licensure and Cleveland Clinic policies.
• Educates patients about disease, treatments, drug choices, and potential side effects.
• Participates in medical rounds and coordinates with nursing and other health team members to ensure quality patient care.
• Refer patients to specialists when needed and ensure continuity of care across teams.
• Contributes to quality monitoring through record reviews and discharge planning, and supports professional development of staff.
• May serve as clinical preceptor and onboard new caregivers.
