Job summary

Gallaher Signature Living in California seeks an AL Wellness Coordinator to lead resident care services in a luxury senior living community. You will ensure compliance with Title 22, coordinate ISPs, and support staff to deliver personalized, dignified care consistent with our hospitality standards.

Reporting to the Wellness Director, you will collaborate with wellness nurses, life enrichment, dining, and operations to promote holistic resident well-being and smooth transitions of care.

Qualifications

• Current LVN certification required.
• High school diploma or equivalent required; AA/AS degree preferred.
• Minimum 2–3 years in assisted living/RCFE or similar.
• Knowledge of California Title 22 regulations.
• Strong organizational, communication, and interpersonal skills.
• Balance regulatory compliance with hospitality-driven, resident-first approach.
• Experience in high-end or luxury senior living preferred.
• Proficiency with EHR or management systems preferred.

Responsibilities

• Coordinate and oversee resident care services in accordance with Title 22 and regulatory requirements.
• Develop, implement, and review Individualized Service Plans (ISPs) based on assessments.
• Observe residents and report changes to Wellness Director and licensed professionals.
• Support residents in maintaining independence while ensuring safety, dignity, and choice.
• Provide day-to-day guidance and coaching to care staff for high performance.
• Assist with onboarding, training, and mentoring of care team members.
• Maintain accurate, timely resident records in compliance with Title 22 and standards.
• Participate in licensing inspections, audits, and corrective action plans.
