Job summary

Piedmont Clinic is seeking an Advanced Practice Provider to join the Thoracic Rescue, ECMO & Lung Transplant Program in a longitudinal transplant-focused role across pre-, peri-, and post-transplant care. The APP will work with transplant physicians and a multidisciplinary team to support comprehensive lung transplant care in ambulatory and inpatient non-ICU settings.

Qualifications

• Current licensure as an Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA-C).
• National board certification appropriate to specialty (APRN/PA-C).
• BLS and ACLS certification.
• Eligibility for credentialing and privileging within Piedmont Healthcare.

Responsibilities

• Perform comprehensive evaluations of patients with advanced lung disease undergoing transplant consideration and assessment.
• Participate in longitudinal management across evaluation, waitlisting, transplantation, and post-transplant follow-up.
• Assist in coordinating transplant evaluations, testing, and multidisciplinary discussions.
• Review laboratory, imaging, and pulmonary function results to coordinate follow-up plans.
• Support immunosuppression management and medication surveillance with transplant team.
• Coordinate transitions of care following hospitalization and transplantation.
• Document accurately in the electronic medical record.
