THIS ROLE IS REMOTE/WORK FROM HOME. APPLICANTS SHOULD RESIDE IN THE GLENVIEW AREA OF COOK COUNTY ILLINOIS.

THE ROLE WILL TRAVEL TO VISIT MEMBERS 50% OF THE TIME, SUPPORTS THE ELDERLY/PHYSICALLY DISABLED MEDICAID LTSS POPULATION.

WORK SCHEDULE WILL BE MONDAY - FRIDAY 8AM - 4:30PM

Position Purpose

Assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes. May develop or assist with developing personalized service care plans for long-term care members and educates members and their families/caregivers on services and benefits available to meet member needs.

  • Evaluates the needs of the member, the resources available, and recommends and/or facilitates the plan for the best outcome
  • Assists with developing ongoing long-term care plans and works to identify providers, specialists, and community resources needed for long-term care
  • Coordinates between the member and/or family/caregivers and the care provider team to ensure services are accessible
  • Provides resource support to members and families for needs like employment, housing, independent living, etc.
  • Monitors care plans, member status, and outcomes, providing recommendations as needed
  • Interacts with healthcare providers and partners to ensure needs are met
  • Maintains documentation for compliance with regulations
  • May perform on-site visits to assess needs and collaborate with providers
  • Educates members and families on procedures, services, and benefits
  • Provides feedback to leadership for quality improvement
  • Performs other duties as assigned and complies with policies
Education/Experience

Requires a Bachelor's degree and 1 year of related experience, or equivalent experience.

Candidate must meet one of the following criteria:

  • RN licensed in Illinois
  • Bachelor’s or Master’s Degree in a human services-related field
  • LPN with one year of experience in assessments and formal service for the elderly
Pay Range

$22.50 - $38.02 per hour
