Centene in Elmhurst, IL is seeking a LTSS Service Care Coordinator to assess and coordinate holistic care management, support members and families, and partner with care teams to improve outcomes.

The role requires a Bachelor's degree (or equivalent), and 1 year of related experience; RN licensed in Illinois or LPN with relevant experience is acceptable. Flexible schedules include remote or hybrid options, with travel up to 50%.

• Assist with developing ongoing long-term care plans/service plans.
• Coordinate between the member and/or family/caregivers and the care provider team.
• Provide resource support to members and their families/caregivers for various needs.
• Monitor care plans/service plans, member status, and outcomes.
• Interact with long-term care healthcare providers and partners.
• Collect, document, and maintain long-term care member information.
• Perform on-site visits to assess member's needs.
• Provide education to long-term care members and their families/caregivers.
• Provide feedback to leadership on opportunities to improve quality of care.

About This Role

The LTSS Service Care Coordinator assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes. This role is ideal for candidates with case management experience and a strong understanding of community-based services.

Highlights

• Pay: $22.94 - $38.79/hour
• Location: Elmhurst, US
• Schedule: Full Time, Day shift
• Travel: Up to 50% local travel to support members throughout specified areas

What You’ll Do

Requirements

• Licenses & Certifications: RN licensed in Illinois, OR Bachelor or Master’s Degree in a human services related field, OR LPN with one (1) year experience in conducting comprehensive assessments
