Job summary

Molina Healthcare is seeking a Field Care Manager (RN) to assess and coordinate care for Medicaid members in Dallas, TX. The role focuses on LTSS populations and delivering quality, member-centered care.

The RN will conduct home visits, develop and monitor comprehensive care plans, coordinate waiver services, and collaborate with caregivers and healthcare professionals to ensure timely access to needed services and supports.

Qualifications

• Active RN license in state of practice (unrestricted).
• Minimum 2 years in healthcare, including 1 year in care management or managed care.
• 1 year working with persons with disabilities or chronic conditions.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitate comprehensive waiver enrollment and disenrollment processes.
• Develop and implement care plans in collaboration with members, caregivers, and healthcare professionals.
• Monitor care plans and evaluate effectiveness, suggesting changes as needed.
• Promote integration of services, including behavioral health care and LTSS.
• Assess medical necessity and authorize appropriate waiver services.
• Evaluate covered benefits and advise on funding sources.
• Coordinate interdisciplinary care team meetings for service approvals/denials.
• Use motivational interviewing to educate and support members.
• Identify barriers to care and coordinate solutions.
