Job summary

Molina Healthcare is seeking a qualified care coordinator to support care management and LTSS activities. You will collaborate with a multidisciplinary team to coordinate integrated member care across the continuum and help members progress toward outcomes.

Travel 25-40% may be required; a valid driver's license and reliable transportation are essential. You will assess needs, coordinate services, and communicate with providers to ensure high-quality, cost-effective care.

Qualifications

• 2+ years healthcare experience with LTSS/care management background.
• Clinical licensure/certification only if required by state contract.
• Valid driver's license and reliable transportation for travel.

Responsibilities

• Completes comprehensive member assessments within timelines, including home visits when required.
• Coordinates waiver enrollment/disenrollment and develops care plans with stakeholders.
• Monitors care plan effectiveness and documents interventions and goals.
• Promotes service integration across behavioral health and LTSS.
• Assesses medical necessity, authorizes waiver services, and advises on funding.
• Leads ICT meetings for service approvals and collaborates with care teams.
• Uses motivational interviewing and Molina guides to support member change.
• Identifies barriers to care and coordinates psycho/social, financial, and medical support.
• Truly supports travel requirements (25-40% local travel).
