Job summary

Molina Healthcare in Cahokia, IL is seeking a care coordinator to support care management and LTSS activities. You will collaborate with a multidisciplinary team to coordinate member care across the continuum and help members reach desired outcomes.

You will conduct assessments, support waiver processes, develop care plans, monitor progress, and facilitate integration of services, including behavioral health, with emphasis on proactive problem-solving and strong communication.

Qualifications

• Requires health care experience with LTSS knowledge.
• Licensure may be required depending on state contract or licenses.
• Valid driver’s license and reliable transportation for travel.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person home visits as required.
• Facilitate comprehensive waiver enrollment and disenrollment processes.
• Develop and implement care plans, including a waiver service plan with members, caregivers, physicians and care network.
• Monitor and evaluate care plan effectiveness and document interventions and outcomes.
• Promote integration of services including behavioral health and LTSS to improve continuity of care.
• Assess medical necessity and authorize waiver services as appropriate.
• Coordinate ICT meetings for approval/denial of services and collaboration.
