Job summary

Molina Healthcare, Inc. is seeking a Care Manager for LTSS to support care coordination and integrated member care. You will collaborate with a multidisciplinary team to ensure high-need members progress toward outcomes while aligning with cost-effective care strategies.

Responsibilities include comprehensive assessments, waiver management, and care plan development, with travel up to 40% depending on state requirements. Strong communication and organizational skills are essential.

Qualifications

• Minimum 2 years health care experience with LTSS exposure.
• At least 1 year in care management or related medical/behavioral setting.
• Valid driver’s license with reliable transportation for travel duties.

Responsibilities

• Completes comprehensive member assessments within regulated timelines and visits as required.
• Facilitates waiver enrollment and disenrollment processes.
• Develops and implements care plans with members, caregivers, and clinicians.
• Performs ongoing monitoring of care plans and notes interventions and outcomes.
• Promotes integration of LTSS with behavioral health and community services.
• Assesses medical necessity and authorizes waiver services.
• Evaluates benefits and funding sources for members.
• Facilitates ICT meetings for approvals/denials of services.
• Uses motivational interviewing and Molina guidelines to educate and motivate change.
• Identifies barriers and coordinates care to address psycho/social, financial, and medical obstacles.
• Collaborates with licensed care managers/leadership as needed.
• 25-40% local travel may be required.
