Job summary

LiveWell Homecare Agency is seeking a licensed Physical Therapist in Georgia to evaluate and treat patients primarily in Rehab and LTAC hospital settings. The role focuses on improving patient outcomes through comprehensive evaluation, plan development, and documentation in the EMR.

Qualifications include a Bachelor's degree in Physical Therapy (Doctoral degree preferred), at least 1 year of rehab experience, GA licensure or eligibility, and current BLS certification.

Qualifications

• Minimum 1 year experience in rehab, SNF, LTACH, or acute care settings preferred.
• Licensed or eligible for licensure as a Physical Therapist in Georgia.
• Current BLS certification.

Responsibilities

• Conducts initial and discharge evaluations, develops treatment plans, and documents progress in the EMR.
• Sets treatment goals and delivers therapy to meet functional objectives in a timely manner.
• Educates patients, families, and staff as needed, and communicates patient progress during clinical conferences.
• Collaborates with interdisciplinary team members and participates in skills checks and training.
• Serves as a mentor for interns and manages equipment orders and vendor communications.
