Job summary

LiveWell Homecare Agency in Atlanta, GA seeks a Physical Therapist to evaluate and treat patients in Rehab and LTAC hospitals. You will develop and implement treatment plans, document progress, educate families, and coordinate with physicians.

Requirements include a Bachelor's in Physical Therapy (Doctorate preferred), at least 1 year in Rehab/SNF/LTAC/Acute settings, and active GA PT license or eligibility. BLS certification is required.

Qualifications

• Bachelor's degree in Physical Therapy is required.
• Doctoral Degree is preferred.
• Minimum 1 year experience in Rehab, SNF, LTACH, or Acute settings preferred.
• Current BLS certification and GA PT licensure or eligibility.

Responsibilities

• Perform assessments, set goals, and create treatment plans aligned with patient needs.
• Deliver timely and effective therapy to achieve functional goals.
• Identify educational needs and provide PRN therapy.
• Collaborate as part of an interdisciplinary team and mentor interns.
• Participate in hospital and discipline-specific skills checks.
