Job summary

LiveWell Homecare Agency is seeking a Physical Therapist to evaluate and treat patients primarily in rehab and LTAC hospital settings, delivering care aligned with patient needs and goals.

The role involves developing treatment plans, implementing strategies to meet functional goals, and collaborating with an interprofessional team. A valid GA PT license or eligibility and BLS certification are required; a Doctoral Degree is preferred.

Qualifications

• Bachelor’s Degree in Physical Therapy required; Doctoral Degree preferred.
• Valid BLS certification and GA PT license or eligibility required.
• Minimum of 1 year experience in rehab, SNF, LTACH, or acute settings preferred.

Responsibilities

• Perform initial and discharge evaluations and develop treatment plans.
• Implement treatment strategies to meet functional goals.
• Identify educational needs and provide therapy education.
• Participate as an active member of the interdisciplinary team.
• Serve as a clinical instructor or mentor for interns when needed.
• Contribute to hospital and discipline-specific skills assessments.
• Manage equipment orders and communicate with vendors.
